I don't even recognize 95% of the names that have one, but they sure seem to have a lot of control over public narrative. Doesn't seem to take much to get one except the "proper" set of views.

I used to have an "anonymous" account there years ago, back when little starlets actually replied to their fans. It was fun to try and make them laugh and answer, or maybe see them post a photo in reply. At some point the famous ones stopped interacting due to stalker fears or other nutcases, which while probably reasonable, made things much less fun. At some point the site demanded a phone number to let me back in, and that was the end of my Twitter user experience.
