I'm sorry to say that at this time it doesn't look like the researcher would be able to move the survey to a different host that doesn't need to have Javascript enabled the way it does on the current surveymonkey host.

We will keep this in mind if we ever share more surveys and will use a different host that works fine with Javascript disabled, such as Google Forms which we could host on our own B4U-ACT Google Apps account.
