I am guessing that you are focusing on the frosting on the cake when it comes to male/female features of a child. Hide the genitalia and then remove hair, clothing and any outward artificial means to distinguish one from the other and it is impossible to tell.
So, take a little boy, make the hair and clothing look feminine and what do you see?
