It could have many different meanings depending on the situation.

You could be with your version of an ideal female and say to yourself,"this is as good as it gets."

Or on the other hand you could be thrown into a stinking rat infested prison cell with soggy moldy bread to eat and your guard (tormentor) says "this is good as it gets."

Or you could you could go to a concert and the group just plain sucks. You ask your friend, "is this as good as it gets?" Then the group picks up and starts to play great, so your friend says "NO, Now it is"
